Maddox was a black firefighter, and the target of bullying and harassment at the hands of his fellow officers at Borough Street, led by Scouser, who nicknamed him "Sooty" and "Monkey Nuts". During a shout alongside Blackwall Blue Watch, Tony Sanderson noticed that Maddox's DSU was missing moments before he was due to enter a building in BA, and Sanderson suspected it wasn't an accident.
Soon after, John Hallam was appointed temporary Station Officer at Borough Street. He became aware of the racist abuse of Maddox and tried to stop it, which only exacerbated the situation. Scouser and the rest of the crew tied Maddox to a ladder and hoisted him over a dam made of hoses. Scouser then kicked the ladder, forcing Maddox's head under the water. Hallam found Maddox and pulled him out of the water, unconscious.
Maddox survived unharmed and was reluctant to take the matter further, not even making a complaint against the other officers. Nonetheless, at the disciplinary hearing which followed, Scouser and the rest were found guilty and dismissed from the Brigade. Maddox' family subsequently had Scouser arrested and charged with attempted murder.
